    Pilot certificate information is available on the FAA website, http://www.faa.gov

    Your Private Pilot certificate shows up there. You may look on the FAA website under Airman Certification and find a link to request a replacement for a lost certificate. Alternatively, you can call 866-878-2498 and talk to the FAA people and request a replacement.

    Great that you still have your log book. You will need to get a flight review per 61.56.
    Get some instruction from an instructor who can then certify the flight review.
    I started flying in 1970. Things have changed considerably since 1968.
